As an Adult Care Services Occupational Therapist, you will hold your own caseload and work in partnership with people living in the community across Hertfordshire to identify person-centred outcomes through holistic assessments.

About the role: Occupational Therapist
You will manage a community caseload, carrying out assessments, care planning and reviews, and liaising with social care colleagues, health partners, housing and care providers to deliver joined-up support.
As part of our Connected Lives practice, you’ll mentor and develop others, champion continuous improvement, and uphold core OT values to help people live the life they want.

Requirements / Skills
- Essential Qualification: Occupational therapy qualification

- DBS Check: Subject to Disclosure and Barring Service clearance
